Minnesota Alliance for Patient Safety

MAPS is a non-profit organization with the mission to “Advance Safe Care Everywhere” and decrease preventable, medical errors.

Medical errors are now estimated to be the third leading cause of death in the United States. 

For over 18 years, MAPS has been the only organization in Minnesota focused solely on safety improvement in health care. MAPS mission of “Safe Care Everywhere” is brought to life by strengthening partnerships between patients/residents/families and care organizations to improve safety. We believe a large part of the answer to safer care is including and engaging patients, residents, and families.

Include Always is a Minnesota movement that brings the conversation of patient safety and medical errors outside the hospital, clinic, and skilled nursing facility walls.

We’ve set out to change the way people think, talk about, and respond to medical errors, as well as inspire action to prevent them. These conversations aim to have our healthcare community members understand why medical errors are a public health issue and how errors affect patients, families, residents, clinical teams, healthcare leaders and payers. When we are all on the same team, we can make a difference to improve healthcare safety.
